How do I submit/make a claim? Show

When your dental treatment is completed, you can download a dental claim form from decaredental.ie. You should complete sections A, B and C of the claim form, and attached your itemised receipt. You can submit your claim using use our secure "Scan and Send" facility, simply log on to http://www.decaredental.ie/scanandsend and follow the simple instructions. Alternatively, you can post your completed claim form and itemised receipt to us: Claims Department, DeCare Dental, IDA Business Park, Claremorris, Co. Mayo, F12 KD85. Please note your claim must be submitted within 12 months from the date of treatment.

How do I get an update on a claim? Show

Claims are assessed within 10 working days, once we have all of the the required information to make the assessment of the claim. To receive an update on the current status of your dental claim, please contact our Customer Service Team on 1890 130 017 / 09493 78608, or email us at query@decaredental.ie.

Where do I find a claim form? Show

Go the the "Claims" section on our website and select the option "Claim form". The dental claim form can be downloaded and printed from here. Alternatively, you can contact our our Customer Service Team on 1890 130 017 / 09493 78608, and we can send you a claim form by post.

How long do I have to claim? Show

All claims must be submitted within 12 months of the date on which the covered dental treatment was completed.

What claim form do I use? Show

You must complete a DeCare Dental claim form which can be found at www.decaredental.ie/claims

What is Scan & Send? Show

DeCare Dental’s Scan and Secure Send facility is a service offered to all members to provide a fast and secure online claim submission option for our members. You can submit your claim online using our "Scan and Send" facility www.decaredental.ie/scanandsend

How do I fill out a claim form? Show

Complete your details in Section A, B and C of the claim form. To ensure prompt payment of your claim, please provide your bank account details and sign the claim form in Section C. Attach your itemised receipt to the claim form. You can submit your claim using use our secure Scan and Send facility, simply log on to www.decaredental.ie/scanandsend TBC and follow the simple instructions. Alternatively, you can Post your completed claim form and itemised receipt to us: Claims Department, DeCare Dental, IDA Business Park, Claremorris, Co. Mayo, F12 KD85. Please note your claim must be submitted within 12 months from the date of treatment.

How do I use Direct Pay? Show

To use our Direct Pay Scheme, you visit a dentist on our Direct Pay network and they settle your bill directly with DeCare. You only pay the balance you are not covered for. You can find our direct pay dentists on our dental directory wwww.decaredental.ie/dentist-finder/, just filter "yes" for direct pay.

What do I need on my receipt? Show

Please submit an itemised receipt with your dental claim form. The itemised receipt should list details of the treatment carried out.

Can I put different treatments on the same form? Show

Yes, you can put different treatments on the same claim form.
